Before we look at the different causes of pulsatile tinnitus, we think it is important to explain what pulsatile tinnitus is.
Pulsating tinnitus can be hearing your heart sounds or pulse in your ear. This can be described as a pulsating flow sound from a vascular malformation or an overactive blood vessel near the ear. It can also be due to contractions in the small muscles of the middle ear. As we mentioned earlier, it is usually said that pulsating tinnitus is a form of objective tinnitus. These sounds can thus be observed and recorded unlike subjective tinnitus. Sometimes your doctor can hear the sound through a stethoscope. Only a few percent of all people with tinnitus have objective tinnitus.

What are the causes of pulsatile tinnitus?
Some other reasons could be:
- Atherosclerosis: A build-up of plaque in the arteries; when this occurs, it can harden and narrow the arterial space, causing a restriction of blood flow. This can cause the characteristic pounding or whooshing sound in one or both ears.
- Sinus wall abnormalities: On the side of the brain is a channel that receives the blood from the veins inside. Certain conditions can cause these ducts to have an increased blood flow which causes the noise.
- High blood pressure: Also known as high blood pressure, high blood pressure can be a cause of the whooshing sound.
- Idiopathic intracranial hypertension: When cerebrospinal fluid builds up around the brain, this can put pressure on the blood vessels.
- Head trauma: One study found that 53 percent of people who suffer a head injury develop tinnitus.
- Paget’s disease: A chronic bone disease involving excessive breakdown and regrowth of affected bones; of people who have this disease also develop hearing problems.
